Year-End Health Checklist: What to Review & Plan for in 2026

As the year comes to a close, it’s the perfect time to reflect on your health. A year-end health review helps you take stock of where you are, what needs attention and how to set yourself up for a healthier 2026. Your family physician can be a great partner in this process. Here’s a checklist to guide your year-end wellness planning.

Schedule Your Annual Physical

If you haven’t had your annual checkup, now’s the time. Preventive visits allow your physician to catch issues early, update immunizations and review your vital signs, lab results and overall well-being. Use this visit to discuss any new symptoms, lifestyle changes or concerns that have developed over the past year.

Review Your Screenings and Tests

Check that you’re up to date on recommended screenings for your age, gender and risk factors. This may include blood pressure, cholesterol, blood sugar, colon cancer, mammograms, Pap smears, prostate checks and bone density tests. If you have a family history of certain conditions your doctor may recommend starting screenings earlier or performing them more often.

Manage Chronic Conditions

If you live with conditions such as hypertension, diabetes, thyroid disease or asthma, the end of the year is a great time to review your treatment plan. Are your medications working? Are your numbers where they should be? Small adjustments now can prevent bigger problems later.

Update Vaccinations

Flu season peaks during winter so make sure your flu shot is up to date. Also ask your doctor about updated COVID-19 boosters, RSV vaccines and any other immunizations recommended for your age group or health status.

Check Your Medications and Refills

Review your prescriptions for accuracy and expiration dates. If you’re changing insurance plans in 2026 verify that your medications are covered under your new plan or ask your physician about alternatives if needed.

Plan for a Healthier Year Ahead

Set realistic and specific goals for the new year like improving sleep, exercising more consistently, eating more whole foods or reducing stress. Your physician can help create a personalized plan and connect you with resources such as nutritionists, fitness programs or mental health support.
